Insurance - Fees

I accept many insurance plans and Employee Assistance Programs (EAP). When you call to schedule an appointment, we will contact your insurance company for you and obtain your particular plan benefits. Also, we will submit the claim forms to your insurance company for billing purposes.


Some of the plans that I accept include but are not limited to:

• United Behavioral Health

• Aetna

• Corpheath – LifeSync

• Bensinger Dupont EAP

• Humana

Additionally, we can negotiate a self-pay fee.
